Comprehensive Field Trip Waiver and Permission Slip

Important: Please read this waiver carefully before signing. By signing, you are agreeing to the terms and conditions stated below.

Waiver of Liability

I, as the parent or legal guardian(Parent/Guardian named below) of (Child named below), a student at Three Cords Academy, hereby release and discharge the Three Cords Academy, its officers, agents, employees, representatives, and volunteers (collectively, the "School") from any and all claims, demands, actions, or causes of action of any kind whatsoever, arising out of or in connection with my child's participation in any field trip or activity organized or sponsored by the School, including but not limited to:

1. Injuries or death resulting from any cause whatsoever, including negligence on the part of the School or its agents.
2. Loss or damage to personal property.
3. Transportation-related accidents or incidents.

Permission to Participate

I hereby give my express permission for my child to participate in all field trips and activities organized or sponsored by the School. I understand that field trips may involve transportation, outdoor activities, and exposure to various environments.

Assumption of Risk

I acknowledge and understand that field trips and activities may involve inherent risks, including but not limited to:

1. Weather-related hazards.
2. Physical exertion.
3. Exposure to allergens or other environmental factors.
4. Unexpected events or emergencies.

I assume full responsibility for any risks associated with my child's participation in field trips and activities.

Background Checks

I acknowledge that the School has conducted background checks on all staff involved in field trips.
